Comments:

MEDERMA cream + SPF 30 sunscreen is non-formulary, not stocked. Defer to outpatient setting or patients may use their own supply.

Use Purelan for Masse Cream.

Mimyx cream is non-formulary, not stocked. A stocked emollient cream or lotion (i.e., Lubriderm lotion) should be dispensed in place of Mimyx.

TETRIX Cream is non-formulary, not stocked. Dispense LUBRIDERM lotion or similar product in its place.

HYLATOPIC is classified as NON-FORMULARY, NOT STOCKED. Lubriderm Lotion or equivalent will be used for inpatients instead. The patient may use their own supply of HYLATOPIC if necessary. Miscellaneous Formulary. Reviewed: September 22, 2009
