Comments:

Qternmet XR is non formulary, not stocked, therapeutic interchange. Dispense as individual components with same daily dosage. Inpatients may use their own medication. 

Dapagliflozin and sAXagliptin will be dosed once daily, metFORMIN may be dosed once or twice daily depending on product used.
