CALOMIST (nasal spray) and the B-12 patch are classified as non-formulary, not stocked. Contact the physician to interchange to cyanocobalamin 250 mcg PO daily (if no GI absorption issues) or 100 mcg IM/subcutaneously once monthly.

Nascobal: If ordered, contact the physician and recommend vitamin B-12 per injection or oral route.
