All doses of conjugated estrogens (Cenestin, Enjuvia) should be interchanged to an equivalent dose of generic Premarin oral tablets on 1:1 mg basis

(Interchange exception: Convert 0.3125 mg doses to 0.3 mg to prevent tablet manipulation).

 

Cenestin, Enjuvia: Discontinued by manufacturers.
