Clozapine REMS Enrollment Instructions

Prior to dispensing clozapine, whether a new start or continuation of home medication and regardless of gender:

1. The Pharmacist must confirm the patient is enrolled in the Clozapine REMS Program.  (For instructions on how to verify enrollment, click on the above link for the Clozaril REMS enrollment instructions.)
2. Once eligibility check complete, create a "Pharmacy Progress Note – REMS" in PowerChart.  Sign the note once all requirements met.  Include in the note the frequency of ANC monitoring from eligibility check, the last ANC submitted, and when the next is due.
