For Treatment of DVT/PE - Pharmacists may automatically enter the missing maintenance regimen (5 mg BID) of apixaban at the time of verifying the initial regimen (10 mg BID x 7 days).

For Non-valvular A-Fib ONLY:
Normal dose: 5 mg BID

Reduce by 50% if 2 out of 3 of the following conditions are met:

  1. serum creatinine >= 1.5
  2. age 80 years or greater
  3. weight 60 kg or less

There is NO reduction in dose for DVT/PE if 2 out 3 criteria are met.

 


For Non-valvular A-Fib OR Treatment of DVT/PE:
Reduce dose by 50% if concurrent therapy with strong inhibitors of CYP3A4 and p-glycoprotein (clarithromycin, ritonavir, etc)
