Interchange for adults at same dose.
Interchange program for 250 mg oral capsules and the suspension is due to national shortage (Sept. 2017).
For the pediatric population, orders of ampicillin 250 mg capsules or suspension will be interchanged to amoxicillin with patient specific dosing by the pharmacist based on weight, height, indication, and other factors.
