THERAPEUTIC INTERCHANGE

omeprazole/clarithromycin/amoxicillin (Omeclamox-Pak)

  • 20 mg-500 mg-500 mg kit oral (Contains omeprazole 20 mg, clarithromycin 500 mg, and amoxicillin 1,000 mg - all dosed BID for 10 days)

pantoprazole (Protonix) AND clarithromycin (Biaxin) AND amoxicillin (Amoxil)

  • ONE 40 mg pantoprazole tablet Daily PLUS ONE 500 mg clarithromycin tablet BID PLUS TWO 500 mg amoxicillin tablets (Total: 1,000 mg) BID – all for 10 days

Omeclamox is classified as non-formulary, not stocked. It should be interchanged to its individual components dispensed separately (omeprazole component should be interchanged to pantoprazole).
