Percocet Interchange for Patients Receiving Scheduled Acetaminophen: Pharmacists may automatically interchange Percocet (oxyCODONE/acetaminophen) to plain oxyCODONE (at same mg dose as oxyCODONE component in original order) at the same frequency in patients receiving scheduled acetaminophen when no acetaminophen free alternative is ordered.
oxyCODONE 7.5 mg with acetaminophen 325 mg- Currently the oxyCODONE 5 mg and 10 mg combinations with acetaminophen are formulary status and stocked. An increase in orders for the oxyCODONE 7.5 mg combination has been reported. This product was approved for formulary status, and the combination with acetaminophen 500 mg should be automatically changed to the combination with 325 mg.
Xartemis XR is classified as non-formulary, not stocked.
