Comments:

HYDROcodone/acetaminophen tablet products containing greater than 325 mg of acetaminophen are being phased out by the FDA. All HYDROcodone/acetaminophen tablet products should be interchanged to the equivalent NORCO product (see Narcotic-Oral Formulary).

LORTAB liquid (7.5 mg-500 mg/15 mL) is being phased out by the FDA. HYDROcodone-acetaminophen oral liquids should all be interchanged over to the new formulary HYDROcodone-acetaminophen liquid product (7.5 mg-325 mg/15 mL) at the same HYDROcodone dose. Reviewed: March 26, 2013

ZAMICET is classified as NON-FORMULARY, NOT-STOCKED OR ORDERED. Interchange to an equivalent dose (HYDROcodone component) of the formulary product (HYDROcodone 7.5 mg - acetaminophen 325 mg/15 mL). If patient is receiving too much acetaminophen, contact the prescriber to switch to a narcotic liquid and acetaminophen liquid administed separately. Reviewed: April 27, 2010 and March 26, 2013.
