Comments:

Norco Interchange for Patients Receiving Scheduled Acetaminophen: Pharmacists may automatically interchange Norco (HYDROcodone/acetaminophen) to plain oxyCODONE (at same mg dose as HYDROcodone component in original order) at the same frequency in patients receiving scheduled acetaminophen when no acetaminophen free alternative is ordered.

HYDROcodone/acetaminophen tablet products containing greater than 325 mg of acetaminophen are being phased out by the FDA. All HYDROcodone/acetaminophen tablet products should be interchanged to the equivalent NORCO product (see Narcotic-Oral Formulary).

Updates: Lortab to Norco Interchange April 2012, Lortab Liquid March 2013 , Zamicet April 2010 and March 2013, Norco to oxyCODONE Interchange January 2026
