FOLOTYN is classified as FORMULARY, not routinely stocked, use restricted to oncology. The case should be reviewed by the oncology clinical specialist or a member of pharmacy administration to evaluate the case for ability to provide the drug in the hospital, reimbursement issues, possible reimbursement strategies, expected toxicities, and other important issues to the case. Drug regimen initiation should occur in the outpatient setting.
