Comments:

LEVOleucovorin calcium (Fusilev) and LEVOleucovorin sodium (Khapzory) is to be interchanged to leucovorin. LEVOleucovorin is dosed at 1/2 the prescribded leucovorin dose.

Khapzory (LEVOleucovorin sodium) is formulary restricted for use when leucovorin injection is unavailable or patient is utilizing Khapzory patient assistance program.

Fusilev (LEVOleucovorin calcium) has been discontinuted by the manufacturer. Khapzory (LEVOleucovorin sodium) is therapeutically interchangeable with Fusilev (1:1 conversion).
