DAPtomycin is restricted to Infectious Disease specialists.


Dose Rounding: Nearest 50 mg increment for patients 18 years and over. Use total body weight in non-obese patients and adjusted body weight in obese patients (obese if TBW/IBW is 130% or BMI 30).

Recommended AdjBW equation for daptomycin: AdjBW = IBW + 0.4(TBW-IBW)
