Orders for Cefazolin every 6 hours will be scheduled as every 8 hours, per care monitoring committee.

Per Anti-Infective Therapeutic Interchange Policy, for preoperative dosing: 

Patient weight greater than 120 kg: use cefazolin 3 gm dose

Patient weight less than 120 kg: use to cefazolin 2 gm dose
