Formulary restricted to:

• Cangrelor orders may only be placed by an interventional cardiologist
• May only be used in patients with no enteral access that require an emergent PCI
• Use in patients that have not been preloaded with an oral antiplatelet drug (including aspirin) prior to arriving in the Cath lab for a PCI
• Bridge therapy:  In patients requiring CABG or other surgery early after PCI when prolonged discontinuation of a P2Y12 inhibitor may not be safe. (maximum of 7 days of Cangrelor infusion in such cases)
