12/8/20 rc Insulin detemir to insulin glargine

The P and T  Committee recently approved pharmacist automatic conversion of insulin detemir (Levemir) to insulin glargine (Lantus) in adult, non-pregnant patients.  The conversion is 1 unit = 1 unit, and given at the same frequency   i.e.   insulin detemir 30 units bid is converted to insulin glargine 30 units bid.   Insulin detemir is restricted to pregnant patients, pediatrics, and no substitution orders.  Because of the pregnancy restriction, an automatic therapeutic substitution cannot be built in Powerchart.  Travis has created an order alert in Med Manager that will notify the pharmacist that the order should be converted to insulin glargine if the patient is a non-pregnant adult.  This is now active in Cerner.  Insulin detemir now costs over 2.5 x the cost of insulin glargine.
