Subject: Pharmacist assistance for TOC technicians

 

Pharmacists,

As TOC continues to expand, there may be times when one of our TOC technicians approaches you with an issue that requires licensure (in order to effectively resolve the issue). The most likely time-frame that this might occur (currently) would be around 5:30-8 pm, M-F.  For example, perhaps a new home med is discovered that was originally omitted from the Wellsoft Med Rec that needs to be addressed by the physician, or maybe a new dose/route/freq/etc. is identified that is different from what was originally reconciled by the physician, etc. 

If you're presented with such a situation, please take a moment to briefly assist them or provide direction as needed. For example, they may need your signature on a communication form to a physician, or you may determine that a communication form isn't necessary if you feel comfortable making the change in iCare based on your clinical judgment, circumstances, etc.
