Subject: Vanco discontinuation reminder

Just a reminder on vancomycin discontinuations, please do the following:

1. Delete the daily consult

2. Create a d/c progress note.  If you forget to do #1, #2 can help us quickly figure out what's going on.  And sometimes we just wonder what happened to patient X.  #2 helps us see what happened.

3. D/C any levels yet to be processed.

Quality is improved if we all share common expectations.  Lately, we've seen a few examples of one or more of these items not being completed.  Lot's of training going on.  Let's all stay together!
