Pharmacists,
 
Dr. Siddiqui will be out of the office starting on Monday July 27th.  Dr. Hassoun will be following all vancomycin patients for his group (Dr. Hassoun & Dr. Siddiqui).  Please sign off of all Dr. Siddiqui's patients starting on July 27th.  Remember to write; "Noted Dr. Hassoun consulted.  Will defer further vancomycin dosing and monitoring to him."  Dr. Hassoun will let us know when to resume monitoring Dr. Siddiqui's vancomycin patients.  I will send out another email to you at that time and we will resume following Dr. Siddiqui's vancomycin patients.
 
Also, the MIDC group has hired a new physician!  Her name is Dr. Paula Debroy.  She has started training with Dr. Gutierrez as of yesterday.  She is able to write for all ID restricted medications.
