Pharmacists,

 

The AMT has developed a new power plan, use criteria, and standard administration time for daptomycin in adult patients.  

  • The daptomycin power plan (HOSP Daptomycin Orders) for ADULTS will help standardize dosing, renal adjustments, and pre-ordered CPK levels.  
  • The new use criteria will address some prescribing issues that were identified in a recent MUE that was conducted by the AMT.  
  • The standard administration time will help minimize PSP waste and improve daptomycin utilization.  

I have attached an instruction sheet as well as a policy regarding this change to this email.  

 

This new process will begin this coming Tuesday (7/14/20).
