Tipsheet - Using Home Med Clarification

HH MAIN pharmacists ONLY:  Home Med Clarification Powerform go-live is planned for Wednesday, July 1, 2020 **Tipsheet attached**

Note: This includes inpatient units and ED Hold/ED Obs at HHMain.  Behavioral Health is excluded from this coverage, as it is classified separately.

 

If you were here prior to Cerner, you may remember the HCS TOC Queue where you could send home med orders that you wanted TOC to clarify.  The idea is the same with the creation of the Home Med Clarification powerform.  It will now be located in the Active Interventions tab of the MPTL along with Pharmacy Interventions.

 

Guidelines to be aware of:

  1. The Active Interventions tab will be monitored by our TOC pharmacists for these new powerforms Monday thru Friday from 7am-5pm. Within this time frame, TOC will address these issues as needed, including making any necessary corrections to inpatient orders.  Any orders that need to be addressed outside of these time frames should be addressed by the unit base/MN pharmacist or clinical specialist as appropriate.
  2. Please use this form to ‘intervene’ on a specific order, and use the powerform to briefly describe the clarificationthat’s needed.  The form should not be used on medications that do not require clarification (e.g. inspection/labeling of a home med).
  3. Please do not use this form to document the home med clarifications that you do routinely throughout your shift.  If this form is present in the Active Interventions tab, we’ll assume you want TOC to take care of it, which will lead to duplicate work if you’re taking care of it also.  Please continue to use the Pharmacy Interventions form as you always have to document the orders you clarify directly with the nurse, provider, etc.
  4. Please do not use this form to request that TOC completely re-do a med history with numerous issues , or if there are multiple medications on a patient to be clarified; instead, please reach out to TOC staff via phone as you always have in those situations.

 

Our hope is that this will help streamline present workflow and improve communication with regard to admission reconciliation issues.  Please feel free to reach out with any questions or feedback you may have.
