**This announcement applies to all pharmacists that dose vancomycin for ADULT patients only**
 
Pharmacists,
 
The P&T committee has approved several updates to our vancomycin dosing guideline and calculator for ADULT patients. In order to familiarize yourself with these updates, all pharmacists that dose vancomycin for adult patients must complete the below CBL and pass the test. Once this is completed, please utilize the new calculator and vancomycin dosing guideline. These are available on Formweb
