Subject: A new Adult Febrile Neutropenia Powerplan is now available

Good afternoon,

Background: A retrospective chart review of 37 adult patients presenting to Huntsville Hospital from January 2019 through December 2019 was performed. Results revealed vancomycin was utilized in a majority of these patients, without documented evidence of clinical need. Of those, the majority of patients were initiated on this regimen in the emergency department (ED).

Plan: Adult Febrile Neutropenia, a new powerplan, has been created and is now available to help reduce exposure to unnecessary antimicrobial treatment in adult patients with febrile neutropenia.

Important information:
Cefepime (Maxipime) will be automatically selected for antipseudomonal coverage. You may make adjustments to this order for patient specific presentation (i.e.-ESBL, penicillin allergy, and/or need for MRSA coverage). 

Vancomycin is not indicated in all patients. It can be considered in the following settings:
a. Suspected catheter related infection  
b. Skin and soft tissue infection 
c. Pneumonia
d. Hemodynamic instability (i.e. – sepsis)
