Hey everyone,

It does appear that moving forward, Surescripts has resolved the issue of inaccurate medications showing in External Rx History (Query) .

 

Again, we believe the issue was occurring between 4/21@1500 – 4/22@1745.  We did our best to wipe out incorrect information for any patients reported, but it is still possible there are patients in our System that have incorrect External Rx History from that time frame. 

 

Please e-mail me or text me any FINs ASAP if you discover this is the case.

 

An example of how wrong the info could be:

  1. For one patient, External Rx History displayed prescription fill information for 6 unrelated people living in 4 other states.

 

As always, External Rx History is a tool to help review home medications, it does not replace the importance of confirming all fill information with patients for accuracy.

Please be aware that Surescripts (reporting agency for External Rx History) has confirmed an issue which began at 4/21/20 @ 3pm affecting their reporting, causing med queries to show very inaccurate reports. As of 4/22/20 @ 5:45p, this has been resolved, per Surescripts.

 

In the event that staff members were to review the External Rx History and import medications over, it is a possibility that medications the patient has never been on could be added to their profile unknowingly, as it would appear they have had it filled previously.

 

Please be aware of this issue if you have been utilizing the Import feature. Please review any patients you may have added medications to by using Import in the past 3 days and confirm they were truly active for the patient. Any patients completed by TOC were reviewed with this reporting error in mind, so they should be unaffected.

 

It is unknown how many patients were impacted by this reporting error, so if you have any questions about this, please feel free to reach out to me and I will be glad to help.
