Pharmacists,
 
I wanted to give everyone an update on the currently available PowerPlans for our COVID-19 patients.
  1. Hydroxychloroquine - **Active Now**
    • All orders for COVID-19 (+) patients will need to be entered using the "ID COVID-19 Hydroxychloroquine (PLAQUENIL) Adult Orders" PowerPlan 
    • This will allow for the correct dose and duration of therapy to be ordered for this patient population
    • Hydroxychloroquine 400 mg PO q 12 hours x 2 doses then 200 mg PO q 12 hours x 8 doses
  2. IL-6 Medications - **Active Now**
    • The use of IL-6 medications is currently restricted to ID physicians and must comply with a use criteria
    • All orders for COVID-19 (+) patients will need to be entered using the "ID COVID-19 Sarilumab/Siltuximab/Tocilizumab Consult to Pharmacy" PowerPlan 
    • When a physician orders one of these medications, it will generate a consult to pharmacy
    • If you receive this consult, please notify AMT (5-0286) or the Hotline after-hours (256-469-8560)
    • AMT will work with the prescribing physician to determine if the patient meets criteria to receive this medication as well as agree upon a dose based on our current medication availability
  3. Azithromycin - **Coming Soon**
    • Use of azithromycin in COVID-19 patients is controversial
    • We are developing a community acquired pneumonia (CAP) PowerPlan (ID Intravenous Azithromycin for Community-Acquired Pneumonia) to help facilitate the current recommended dosing with appropriate duration of therapy for both CAP and COVID-19
    • Azithromycin 500 mg IV x 1 dose then 250 mg IV q 24 hours x 4 doses
