Hey all,

All of last week providers were reminded to enter Fentanyl patches through the HOSP fentaNYL (DURAGESIC) Patch Orders PowerPlan via their Cerner splash screen, and they were sent their own version of the tip sheet: https://docushare.hhsys.org/dsweb/Get/Document-84163/Tip%20Sheet%20-%20Fentanyl%20Patch%20Orders%20-%20Provider%20Version.pdf.  If you encounter additional orders for Fentanyl patches outside the PowerPlan, please contact me directly and I can forward the provider to Phys Track team for further education.

Quick note for inpatient pharmacists:

All inpatient orders for Fentanyl Patches must be entered through the HOSP fentaNYL (DURAGESIC) Patch Orders PowerPlan. Fentanyl patches should never be ordered or verified standalone, even if attempting to continue a home medication

During Order Verification, look for the PowerPlan icon (pizza box) for HOSP fentaNYL (DURAGESIC) Transdermal Patch Orders

If the order is not through the PowerPlan, either enter the appropriate orders via the PowerPlan through PowerChart or call the provider to do so, then VOID the inappropriate order.

A Sentri7 rule exists to help pharmacists ensure these orders are appropriately placed.

Please whenever you have a chance review the details behind this in the master Cerner tip sheet on Formweb (currently page 63 on Fentanyl Patch Orders – PowerPlan only): https://docushare.hhsys.org/dsweb/Get/Document-80881/Cerner%20Tipsheet%20Master.pdf
