Subject: New Pregnancy Assessment Guideline - Go live 2.17.20
The Pregnancy Assessment guideline has finally completed the committee tour and is posted on PULSE:
https://docushare.hhsys.org/dsweb/Get/Document-83402/Pregnancy%20Assessment%20for%20Pharmacy%20Verification%20of%20Medications.pdf
Beginning tomorrow, 2.18.20, pregnancy status will be a required field on the nurse's admission assessment.
1. Status is documented as N/A, confirmed positive, confirmed negative, patient denies, or patient states pregnant or possibly pregnant
2. N/A is used for males, hysterectomy, pre-pubescent, post-menopausal (> 1 yr without periods), or tubal ligation (reference text with this list has been added to the nurse's screen)
3. For 'patient denies', nurses are asked to document the LMP; if < 28 days since LMP (begin date), treat as not pregnant - pregnancy tests are not accurate during this time period
4. A pregnancy test may be requested if needed to clarify pregnancy status for medication verification - contact nurse; he/she will enter an order as 'no cosign required'
5. A pregnancy-medication conflict alert will fire if either 'confirmed positive' or 'pt states preg/possibly preg' are selected and the medication is on the pregnancy concern list within Cerner
6. Additional fields have been added to the Pharmacist View in Med Manager: Infant Feeding, Newborn Intake, and LMP - see attached screen shot
