Subject: Weekend Hospice On-Call Pharmacists

Starting 1/4/20, we will have on-call pharmacists for Hospice Family Care during the WEEKENDS (S-S) ONLY. If you receive a call from Hospice, please direct them to the attached on-call calendar (Hospice has a copy as well). Issues could involve emergent meds for an existing or new Hospice patient over the weekend, Hospice Pyxis problems, etc. Let me know if you have any questions or concerns.
