Recent audits have demonstrated improved documentation of IV Contrast administration, and we are comfortable with moving forward with the plan to stop faxing IV Contrast warning documents to Pharmacy effective Monday, December 2nd.  Below is previous communication on this topic and attached is a review of the rules in place to support the elimination of the paper fax for reference.

The original plan was to have Radiology/Imaging/Cath areas stop sending the faxed IV Contrast warning documents on Friday of last week (8/9). 

However, an audit of faxed notifications showed that the documentation of contrast administration was not occurring 100% of the time.  Since the rules in place depend on administration documentation, the faxes will continue to be sent to Pharmacy for review and will be re-evaluated in the coming weeks.  Thank you for your patience on this.

Later today, two new rules related to Metformin-IV Contrast interactions will be activated in 1Chart.  Attached is a reference document detailing these new rules.  Of note, Radiology/Imaging will still continue to fax the handwritten forms indicating IV Contrast administration through the end of next week (Friday, August 8) while we confirm the rules are working appropriately.  I will send out another message once these are live (likely around NOON today), but wanted to give everyone a heads up about them.
