Subject: Re: IV Acyclovir Back Order

Pharmacists,

IV acyclovir is on shortage and is expected to last until at least December 2019 or January 2020. The supply of IV acyclovir is expected to run out very soon.

Effective immediately, the use of IV acyclovir will be restricted to specific situations and closely evaluated by clinical pharmacists and the Antimicrobial Management Team (AMT). Potential alternatives to IV acyclovir include: acyclovir PO, valacyclovir PO,  ganciclovir IV, or foscarnet IV.

All pharmacists will determine if IV acyclovir meets the attached criteria for use prior to order verification. If IV acyclovir does not meet the attached criteria for use, the pharmacist will contact the ordering prescriber for alternative recommendations.

Please see attached treatment recommendations for conserving use of IV acyclovir during the shortage.

Let me know if you have any questions.
