Subject: Vaccine Charting Reference




We have had a number of calls to Pharmacy from Nursing regarding encountered 
issues with charting Vaccine doses. Vaccines require additional charting 
elements that a Nurse may not be used to completing. I have attached a quick 
reference for Pharmacist use in helping troubleshoot/walk through the 
vaccine charting process. Ultimately, if this does not assist in resolving 
the charting issue, please have the Nurse contact the IT Help Desk. There 
should be a Point of Care IT contact on call who should be able to remote in 
and assist the user with charting.
