Subject: Dexmedetomidine 400 mcg/100 mL change

On Tuesday, October 1, Huntsville Hospital will be converting from Dexmedetomidine 400 mcg/NS 100 mL to Dexmedetomidine 400 mcg/D5W 100 mL.  The NS product will be removed from all Pyxis machines and replaced with the D5W product.  Following conversion on 10/1/19, all new orders will be entered into Cerner as the D5W formulation.  To facilitate Pyxis removal, any old active orders will need to be switched to the new product after the Cerner change is complete.

If medically necessary, the NS formulation can be entered through the **Pharmacy Only** infusion Powerplan, and PSP will dispense those doses. The NS formulation will no longer be available in the Pyxis machines after the switch is made.

The remaining NS product removed from Pyxis will be delivered to PSP, and PSP will dispense the medically necessary NS doses.  The premixed NS product can be used until the supply is exhausted, but once we have exhausted this supply, PSP will mix all NS doses. Keep in mind, the usage will be MUCH lower than when PSP mixed Dexmedetomidine in the past, since the pre-mixed D5W product will be used for most doses.
