Yesterday, I sent out info on opioid stewardship. Today's topic is ANTICOAGULANT INITIATION and MAINTENANCE. I have attached the policies for orals (including DOACs), heparin drips, and LMWH.
Please review.
National Patient Safety Goal 03.05.01 requires that we reduce the likelihood of adverse events with anticoagulants. You are well aware of many of these actions (baseline INR for warfarin, warfarin-food/drug interaction monitoring, programmable pumps for heparin drips, patient/family education), but please read the attached guidelines in case you are asked about our anticoagulation protocols during a tracer.
