Subject: **Attn "Q" Pharmacists** HS Rehab: Dialysis Patients

 

 

Beginning next week, HealthSouth Rehab will be transitioning to Cerner as their eMR. This transition will primarily impact the Q area and any other Pharmacist position verifying for Rehab patients while they are receiving Dialysis treatment.

 

Important Notes:

 

-iCare accounts for Rehab patients will still exist. However, standing medications orders will not exist in the patient’s iCare profile. If any medication is needed for a patient during Dialysis visit (including standing orders from Rehab), the RN will obtain a new ONCE verbal order from nephrologist. This order will then be recorded, entered into iCare, and faxed to Pharmacy for verification/distribution. These patients will still show in Pyxis, so loaded items will still be able to be removed after verification. Otherwise, we will handle the order as it is done today (printing to distribution area for prep and distribution to unit). This process is no different, but you may see unique orders for verification outside of the typical, such as insulin or pain medication.

 

-Profile information in iCare will be limited. We’re uncertain at this time how much information may be maintained in iCare for these patients. If you need further information prior to verification (Ht, Wt, Allergies, Pregnancy/Lactation) which is unavailable in iCare, please reach out to Dialysis directly. Rehab is meant to provide H&P, patient orders, and MAR to Dialysis unit upon transfer, and should be able to fax these documents for Pharmacy review if needed.
