Subject: Vancomycin - Dosing Responsibilities

Good Morning,

Just a reminder.....when you receive a vancomycin order (either automatic or consulted), please enter the dosing regimen (not just a one time dose). This prevents confusion for the pharmacist that is following up on the consult and it also ensures that the patient receives enough doses to cover them until follow-up. Please let me know if you have any questions.

Thanks,

Mary
