Subject: Communication regarding safety events

 

 

As many of you know, the hospital holds a daily "Safety Call" as part of our transformation to a Highly Reliable Organization. This call generally covers important QRRs, including errors, falls with injury, employee accidents/exposures, and more. It also features pertinent announcements, such as malfunctioning equipment that affects other departments, important drug shortages, security issues, etc.

 

 

One of the expectations of this daily call is that the leader representing each department give a synopsis of issues in their area: what happened, whether or not it reached the patient, whether or not it caused patient harm, and whether or not the patient/family and prescriber were notified. In most cases, if the QRR is entered by Pharmacy personnel, the description given is generally sufficient for the leader on call to give a quick synopsis. In cases where Pharmacy or medications were involved but the QRR is entered by another department, there may not be enough information for the on-call Pharmacy leader to respond. In situations where you are aware that there was a QRR entered (or other safety issue exists) involving Pharmacy or meds but you are NOT the one entering the QRR, please send along any other info of which you are aware to a new group email group: pharmacy.safetycall@hhsys.org

 

 

Most QRRs will not require this follow-up email, but if more information needs to be communicated to the Pharmacy leader on call, please send us a quick email with more details (see needed info above). Please be sure to include patient name, unit/dept, and medical record number.
