Learned something new today ....

Please VOID the dummy order of 'Vancomycin-pharmacy to dose' through Pharmacy Medication Manager after entering the Vancomycin dosing in PowerChart and confirming the "Vancomycin PKS Dosing Consult" is active in PowerChart consults.

Voiding prevents the 'Vancomycin-pharmacy to dose' from hanging around on the active medication list in PowerChart.

Discontinuation may leave the dummy order for 'Vancomycin-pharmacy to dose' on the PowerChart medication profile.

Subject: Vancomycin - pharmacy to dose

 

When prescriber has discontinued Vancomycin on a patient, please REMOVE the "Vancomycin-pharmacy to dose" from the Med Manager or via PowerChart.

This order is causing confusion for the prescribers as they are learning the system leading to prescribers believing the patient has gram positive coverage.

In the situation I was alerted to today, the "Vancomycin-pharmacy to dose" has been hanging on the profile since 3/11 when the drug was discontinued. Two different groups of physicians 'thought' the Vancomycin was active.

If you are unsure of removing the "Vancomycin-pharmacy to dose" from the profile, please notify the covering pharmacy clinical specialist.
