Hey everyone,

 

For reference, attached is the Pharmacist Clarification of Home Medications guideline and its quick reference to guide what pharmacists can and cannot adjust Per protocol aka “No Cosign Required” for inpatient orders continued from the home med list. Although approved prior to Cerner, the content still applies.

 

Of course pharmacist judgement always take precedence, but please also update the patient’s Cerner home med list (Document Medication by Hx) if you happen to correct a discrepancy.
