02/14/19 AA -- Vancomycin 1250 mg and 1500 mg

Subject: Vancomycin 1250 mg and 1500 mg

There was an update applied this afternoon so that orders entered through Powerchart for Vancomycin 1250 mg and 1500 mg IV Piggyback, it should route to a ‘PREMIX’ in Normal Saline (NS) one line item vancomycin product as shown below.   Additionally, when selecting a Vancomycin order for these 2 doses directly in MedManager, please utilize these PREMIX one-line items instead of an IV Set. 

Note: Though it states ‘PXS’ on this item to designate as Pyxis loadable, it will STILL be dispensed from the IV Room and NOT loaded in Pyxis devices on the floor.

If you need to change the fluid to D5W, please select the vancomycin ___ mg IVPB IV Set for now.  You will need to update the IV set to remove the fluid and change it to D5W.
