Subject: Adult Pneumonia Orders (Pulmonary) in iCare - PKS consults

A wording choice has lead to some confusion on the order set [ Adult Pneumonia Orders (Pulmonary) ]

For the 2 different PKS consults, please enter further dosing besides just the original bolus/first dose:
tobramycin 6 mg/kg ( _________mg ) IVPB x 1 (Consult PKS for dosing)
vancomycin 19 mg/kg ( ____________mg ) IVPB x 1 (Consult PKS for dosing)

The intention of the order set was to give to a more appropriate first dose by the order set, followed by PKS consult to continue dosing. However, this has been interpreted at times to be a single dose of Vancomycin.
