Subject: Pod G HHM Vancomycin

When Pod G was first transitioned into being an observation unit, the decision was made to not do automatic consults for vancomycin unless requested by the observation physician. Starting next week we will handle any order for vancomycin from G pod the same way we manage vancomycin orders from anywhere in the ED. If a pharmacist receives an order for vancomycin from G pod they should make sure that the patient has already received a dose or will receive an initial dose and place that consult on the consult list in iCare. A clinical specialist (typically the ED pharmacist) will make a folder for that patient, add them to the NSS/PKS list, and follow that patient until they are admitted or discharged. On weekends the folders will be given to the N.
