Subject: IV fluconazole

 

 

We are almost out of IV fluconazole - maybe a week's worth left. At this time, we need to switch all possible IV fluconazole patients to PO (this is an already-approved P&T conversion) and make sure that all patients ordered fluconazole are started on PO if they are able to swallow it. If the patient is unable to take PO, they will have to be switched to an alternate product...probably micafungin. You would need to obtain physician approval for a switch to an alternate drug. The ID Pharmacy team is in contact with the ID physicians to let them know of the situation and you are welcome to call Jonathan or Logan if needed.
