Subject: IV diphenhydramine

Thanks for reviewing Sentri-7 and medication profiles. We are down to 11 patients with an active order for IV diphenhydramine...please continue to review and switch to PO unless the patient is being treated for a severe/anaphylactic reaction. If they are unable to take PO, the physician should be contacted for an alternate agent for itching, allergic rhinitis, insomnia, or other non-life-threatening symptom.

IV diphenhydramine will be removed from most Pyxis machines today; exceptions include EDs and central Pyxis machines in OR areas. Going forward, pharmacists should only verify orders for IV diphenhydramine in severe/anaphylactic reaction situations.
