Pharmacists,

You must write your initials on all patient-specific pharmacy-prepared products. Patient-specific meds are NOT documented in the unit-dose log book or narcotic unit dose log book. Only the tech's initials are on our labels. It is not okay for patient-specific doses to have only the tech's initials (i.e. 'original bottle' doses, patient-specific oral liquids, etc.).

Any NON patient-specific unit dosing CAN have only the tech's initials because the pharmacist signs off on those in the unit dose log book (or narcotic unit dose log book).

All doses must be traceable back to a pharmacist's check.
