More shortages - please see below regarding SODIUM PHOSPHATE...please remove from adult TPNs beginning Tue, Oct 9.

 

Also, please remember that we are critically short on IV diphenhydramine (convert to PO), Na bicarb injection (see FormWeb and emails for restrictions), and hydralazine injection (labetolol may be an option).

Can you send notification to the pharmacists to start tomorrow - no sodium phosphate in TPNs or IV sodium phos boluses for adults. Potassium phosphate is in stock and can be used at this time for both. Thanks
