Medications not yet evaluated by P&T are considered NON-FORMULARY . . . . . Always check 2 unique patient identifiers - NAME and DATE OF BIRTH - at every step! . . . . . Please be sure to document all clinical activities daily.

1/2/2017 -- Formulary Documents on Docushare

Because it is not feasable to input every drug that has been addressed (antacids, lotions, etc.), a link has been added to docushare where these less common names are searchable-- assuming they have been addressed by P&T and are on formulary document. 

From the home page click on the "Pharmacist" button, at the top right under "Pharmacist Resources" is the new link titled "Master List of Formulary Documents on Docushare."
