Subject: QRG Antibiogram Correction

Pharmacists,

Please be advised the antibiograms in the QRG are off by one line. This is due to the change in our reporting platform moving away from doripenem and adding meropenem. Starting on page 33 and ending on page 36 please make the following adjustments to the antibiogram tables on these pages.
  1. Delete the word doripenem from the box on the left of the chart
  2. Move antibiotic names up one line starting with erythrormycin and ending with linezolid
  3. Add meropenem to the line vacated by linezolid

This will bring the data to the right of the medications in line with the appropriate medication.
Very sorry for this oversight. Please let me know if you have any questions.
