Subject: Intervention/activity documentation

Please be sure to document your clinical interventions and activities thoroughly. Our numbers are down significantly, including a 33% drop in activities documented in POS/Quantifi.

I know that everyone is extremely busy and feel that this is probably due more to lack of complete documentation rather that a drop in interventions or activities. Please dedicate a little time each day to make sure that your documentation is complete and that all workload is captured.
