Subject: Kcentra - dose rounding

Kcentra is currently supplied in 500 unit and 1,000 unit vials. Please round all adult orders to nearest 500 unit increments.

The protocol reads "rounded to the nearest available vial size by pharmacy." Please do not verify adult doses that are not in the nearest 500 unit increments.

Please let me know of any issues. Thanks.
