Subject: new CRITICAL SHORTAGE - famotidine injection

We have a critical shortage of famotidine 20 mg injection. We are hearing anywhere from late May to mid-summer before the supply returns to normal.

We need all pharmacists to take the following action immediately :

    * switch all appropriate patients to PO famotidine (please consider this switch to PO first-line if the patient meets criteria)
    * switch to IV pantoprazole at an equivalent dose if the patient requires parenteral acid suppression
